The mean velocity of the vertebrobasilar system among central vertigo patients

The central vertigo differs to peripheral vertigo due to vascular disturbances such as vertebrobasilar artery insufficiency. This difference is represented by the mean velocity of the arteries measured using Transcranial Doppler Ultrasound (TCD).

A study involving 19 central vertigo and 34 peripheral vertigo cases was conducted to compare the mean velocity of the basilar right and left vertebral arteries; considering sex, age, blood pressure, blood sugar, blood cholesterol, hemoglobin, and haematocrite as factors related to the hemodynamic of the cerebral blood flow.

The diagnose of the type of vertigo was based on the clinical features and examinations. This was then followed by Ct-scan examination among those central vertigo cases to verify the occurrence of structural brain damages that related to the vascular disturbances.

There is no significant different of the age of the central vertigo and the peripheral vertigo group; with difference proportion of male vs. female between the two groups: 2:1 and 3:1 respectively. The systolic and diastolic were significantly different (p: 0054; p: 0153). There was significant different of the mean velocity of the basilar, right and left vertebral arteries between the two groups. Among the peripheral vertigo cases, the right vertebral arteries were significantly different to the left one. Central vertigo caused by cerebral blood flow disturbance (ischaemia or hemorrhage) showed a decreased mean velocity of the vertebrobasilar arteries, even though they were not significantly different to the peripheral cases. It was found that the mean velocity of the male basilar artery among the total vertigo cases was higher than the female (p: 0,017); the mean velocity of the artery was higher among under 40 years of age group than the older one (p: 0,038). In term of sex, blood pressure, blood sugar, blood cholesterol, hemoglobin, and haematocrite, there were no significant different of the mean velocity of the basilar artery. Using a multiple regression test, it was found that blood cholesterol and sex have significant correlation to basilar artery's mean velocity (p: 0,021 and p: 0,005). It can be concluded that some factors influence the mean velocity of the vertebrobasilar arteries, but did not produce differences of the arteries' mean velocity between the central vertigo and the peripheral vertigo group.
